About The Position

Brentwood Behavioral Healthcare is seeking a dynamic and talented Patient Care Coordinator for their Outpatient Department. This role involves working with other providers to ensure the implementation of individualized service plans through appropriate linkages, referrals, coordination, and follow-up to needed support services and direct care services. Specific activities include assisting patients in their community, follow-up care to assess adherence to medical recommendations, assisting patients with travel to important meetings and appointments, and helping patients manage their multiple health issues. The position utilizes an integrated approach where the Peer Support Specialist uses their own experiences to offer individuals assistance in achieving stability and obtaining and maintaining coping and basic life skills consistent with their treatment goals. Peer Recovery Support Services are an evidence-based model of care delivered by a qualified peer support provider who assists individuals with their recovery, potentially through in-person, telemedicine, or audio-only methods. The role requires availability for Full Hospital and Departmental orientations, Monday - Friday, 8 AM - 5 PM.

Requirements

  • High school diploma or equivalent.
  • 1-2 years experience in a mental health field (psychiatric/inpatient or outpatient setting), residential or group home setting.

Nice To Haves

  • Bachelor’s Degree in Mental Health related field.
  • Two years of experience in the mental health field (psychiatric/inpatient or outpatient setting).
  • 1-2 years experience in a psychiatric or CD inpatient, residential or group home setting.

Responsibilities

  • Ensure implementation of the Individualized service plan (with active patient involvement) through appropriate linkages, referrals, coordination, follow-up to needed support services, and direct care services.
  • Assist patient in their community.
  • Conduct follow-up care to assess adherence to medical recommendations.
  • Assist patients with travel in reaching important meetings and appointments as indicated in the treatment plan.
  • Assist patients with managing their multiple health issues.
  • Utilize own experiences to offer individuals assistance in achieving stability and obtaining and maintaining the level of coping and basic life skills that is consistent with their treatment goals.
  • Deliver services in-person or through telemedicine or audio-only.
