About The Position

The Care Coordinator provides case management services in a manner consistent with the philosophy and objectives of Cedar Springs Hospital for his/her assigned caseload. He/She serves as the liaison between the facility Treatment Team and the patient's support person(s), collaterals, etc. The Care Coordinator is the primary responsible party for discharge planning in the outpatient department and is responsible to ensure the patient's discharge is a safe and seamless transition to a less restrictive level of care. To perform the job successfully, an individual must be able to demonstrate competency in the criteria listed in this job description. The specifications listed below are representative of the knowledge, skills, and ability to meet the minimum requirements for this position. Staff must demonstrate flexibility for reassignment to units providing care to varied populations including children, adolescents, adults, and dually diagnosed patients in an outpatient setting.

Requirements

  • Bachelors degree in Human Services or related field required.
  • Three years prior experience working with a psychiatric mental health population required.
  • Ability to consistently meet and manage patient expectations.
  • Ability to provide timely and appropriate care specific to patient needs.
  • Ability to provide care appropriate to patient's developmental level and needs.

Nice To Haves

  • Masters Degree preferred.
  • Experience in an outpatient psychiatric setting preferred.
