About The Position

This position is responsible for facilitating meetings, collecting data, reporting on progress, assisting with project tasks, and conducting accountability follow-ups to implement priority health area projects. The role involves increasing community partner engagement and collaboration, coordinating health events and presentations, and facilitating training opportunities for staff and community partners. Additionally, the position will disseminate information regarding training, resources, events, grant opportunities, and health care access and quality. The role also includes providing individualized assistance to clients to overcome healthcare system barriers and facilitate timely access to quality screening, diagnostics, and treatment for women diagnosed with cancer.

  • Knowledge of privacy laws to protect confidentiality, including Health Insurance Portability and Accountability Act and applicable state and local privacy laws

Responsibilities

  • Facilitate meetings, collect data, report on progress, assist with project tasks, and conduct accountability follow-ups to implement priority health area projects.
  • Lead or co-lead at least four quarterly (CHD internal) meetings and activities.
  • Increase the number of community partners and/or level of collaboration in projects.
  • Coordinate/participate in at least four activities/events that support the improvement of residents’ health status.
  • Coordinate two presentations to Coalition members, updating them on the projects’ progress, issues encountered, partner recruitment, and data tracking.
  • In collaboration with OMH, facilitate at least three training opportunities for health department staff and community partners to increase their capacity to address health priority areas.
  • Disseminate information to the appropriate partners regarding training, resources, events, grant opportunities, health care access and quality, best practices, etc. monthly. Maintain documentation of requested and disseminated resources.
  • Maintain a CHD Team consisting of staff from various program areas.
  • Host/participate in at least four promotional health events to connect individuals to community resources, increase partnerships, and enhance collaboration.
  • Maintain a County Coalition and Plan Taskforce to guide and implement health projects to address the priority areas.
  • Provide individualized assistance to clients to help overcome healthcare system barriers and facilitate timely access to quality screening and diagnostics as well as initiation of treatment for women who are diagnosed with cancer. This includes but is not limited to: Conduct patient and provider call, send referrals to the appropriate facilities, completion of the patient report form (PRF), tracking enrollment for screening, re-screening, diagnostic, and treatment services.
