About The Position

Baylor St. Luke’s Medical Center is seeking an Oncology Clinical Pharmacist Specialist to serve as a critical, real-time medication expert. This role involves providing immediate, life-saving pharmacotherapeutic interventions for acutely ill and critically injured patients. The specialist will optimize drug therapy, mitigate medication risks, and lead medication management during high-stakes emergencies, directly impacting patient outcomes. Responsibilities include providing expert pharmacotherapeutic consultation and direct patient care during resuscitations, trauma activations, stroke alerts, and other medical emergencies, making rapid, evidence-based recommendations on drug selection, dosing, administration, and monitoring. The role also involves managing complex toxicology cases, leading medication preparation in critical situations, and providing urgent drug information to physicians and nurses. Additionally, the specialist will contribute to quality improvement initiatives, precept pharmacy residents and students, and provide education to emergency department staff. Success in this role requires specialized training in emergency medicine pharmacy, clinical acumen, and composure to manage complex pharmacotherapy in high-stress environments, collaborating with physicians and nurses as the definitive medication expert.

Requirements

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree
• Residency completion or three (3) years of clinical experience
• Active Texas Pharmacist License in good standing
• BLS Certification issued by the American Heart Association
• ACLS Certification issued by the American Heart Association

Responsibilities

• Provide expert pharmacotherapeutic consultation and direct patient care during resuscitations, trauma activations, stroke alerts, and other medical emergencies.
• Make rapid, evidence-based recommendations on drug selection, dosing, administration, and monitoring.
• Expertly manage complex toxicology cases.
• Lead medication preparation in critical situations.
• Provide urgent drug information to physicians and nurses.
• Contribute to quality improvement initiatives.
• Precept pharmacy residents and students.
• Provide education to emergency department staff.
• Perform all duties of a registered staff pharmacist working online/staffing.
• Promote long and short-range planning to respond and integrate within the market, Region, and systems.
• Work collaboratively with pharmacy peers to standardize clinical pharmacy systems when appropriate.
• Develop, coach, and mentor pharmacy staff to maximize their scope of practice in the provision of clinical pharmacy services and scholarly activities.
• Educate medical staff regarding cost-effective, quality pharmaceutical care based on evidence-based medicine and best practices.
• Participate in assigned committees relevant to their knowledge, experience and practice area.
• Assure that clinical pharmacy initiatives are in compliance with regulatory requirements.
• Precept Pharmacy Interns and Pharmacy Residents.
• Function as a clinical pharmacist (centralized/ decentralized staffing).
• Demonstrate ability to document clinical activities and interventions such as consults, patient teachings and other patient specific information as defined by policy and procedure.
• Interpret medication orders and/or prescriptions for drug interactions, appropriate drug selection and appropriateness.
• Accurately enter the order or prescription into the computer system, label, and dispense medications in a timely manner.
• Verify and check medications prepared by other pharmacy personnel.
• Communicate drug information to patients and health professionals.
• Resolve drug-related problems by consulting with other healthcare professionals to achieve appropriate patient outcomes.
• Clarify unclear orders written by a licensed provider with the licensed provider.
• Assist the licensed provider in appropriate drug selection and therapeutic regimens.
• Achieve and maintain an adequate basic knowledge base to provide clinical services for all age groups served.
• Provide basic clinical services as required by policy and procedure.
• Maintain a working knowledge of automated systems.
• Responsible for technician activities, performance, provides direction and guidance as needed.
• Participate in the orientation, instruction and training of pharmacy students and interns, pharmacy technicians, residents, pharmacists and other health professionals.
• Perform duties in documented accordance with requirements of all regulatory and governing bodies.
• Complete mandatory education from a system and department level.
• Provide patient counseling and drug information and documentation as appropriate.
