The OCDD Support Coordinator Supervisor is responsible for overseeing the delivery of support coordination services. This role involves reviewing plans of care, making initial contacts for new linkages, assisting support coordinators with various documentation and corrections, and ensuring compliance with standards and quality of service delivery. The supervisor will conduct regular case record reviews, hold weekly individual sessions with support coordinators, manage caseload assignments, and accompany support coordinators on home visits. Additionally, the supervisor will arrange training, direct staff to meet outcomes, provide feedback on service delivery, teach person-centered principles, assure quality, review timesheets and mileage reports, and handle discipline, coaching, and annual performance reviews for direct reports.
