About The Position

FOX Rehabilitation is seeking a licensed Occupational Therapist to provide one-on-one care to older adults in Pooler, Georgia, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Clinicians may also incorporate telehealth and other virtual care into their schedules when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and support patients’ mobility, independence, wellness, and quality of life. FOX describes its practice as clinician-operated and grounded in meaningful relationships, evidence-based care, and a mission to abolish ageism. The position involves collaboration with an interdisciplinary team and education for patients, caregivers, and senior living staff. FOX lists full-time, part-time, and PRN / FOX Flex employment pathways. Full-time and part-time roles are described as salaried, with benefits for eligible roles; PRN / FOX Flex is compensated per unit. New graduates may be eligible for the Emerging Professionals Mentor Program.

Requirements

Valid Occupational Therapy license in the state or states of practice, or eligibility to obtain one.
Degree from an accredited Occupational Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Ability to drive to patient homes and senior living communities.
Wear PPE when required.

Responsibilities

Provide one-on-one occupational therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
