About The Position

Community Care is hiring an Occupational Therapist to support its mission of helping older adults and adults with physical and intellectual disabilities live as independently as possible in the community. This full-time hybrid role serves members in Greater Southeast Wisconsin and involves team collaboration, community case management, clinical consultation, and support for care planning. Regular local travel is required.

Requirements

Bachelor’s degree in Occupational Therapy or Physical Therapy.
Current Wisconsin professional licensure.
A minimum of 3 years of recent clinical experience with adults over 65 or people with physical or developmental disabilities in a hospital, home care, or nursing home setting.
Regular local travel, requiring use of a personal vehicle, a valid driver’s license, and proof of insurance coverage.

Nice To Haves

Master’s degree in Occupational Therapy or Physical Therapy; a Doctorate in Physical Therapy is also preferred.
Strong background in home care, case management, or another aspect of multidisciplinary care management.
Deep understanding of DME and seating/mobility products.

Responsibilities

Consult on members’ needs for adaptive and/or durable medical equipment (DME) and home modifications.
Consult on the need for ongoing skilled PT/OT/ST and assist teams in developing discharge plans.
Serve as a resource to teams in care planning and problem-solving, and provide resources and interventions for members.
Develop and present training to teams, including assisting with new employee orientation.
Perform clinical reviews of authorized skilled nursing and outpatient rehabilitation services and DME quotes.
Document therapeutic interventions in accordance with company and regulatory requirements.
Complete home visits as needed.
Perform other related duties as assigned.
