About The Position
Activa Home Health, an AccordCare Company, is hiring a Home Health Occupational Therapist to provide care in patients’ homes in Palm Beach County, Florida. The role focuses on delivering occupational therapy evaluations, reassessments, and individualized treatment to support patients’ daily living, mobility, strength, coordination, and safety. The therapist educates patients and caregivers, documents care, and collaborates with physicians and interdisciplinary team members while following Medicare and agency guidelines. This is a per diem position with flexible scheduling and competitive per-visit rates. The role requires screening through the Care Provider Background Screening Clearinghouse.
Requirements
- Current and active Florida Occupational Therapist (OT) license.
- Current CPR certification.
- Valid driver’s license and reliable transportation.
- Strong clinical assessment and communication skills.

Nice To Haves
- At least one year of Home Health experience is preferred.
- Medicare Home Health experience is strongly preferred.
- Experience with EMR systems; WellSky/Kinnser is preferred.
Responsibilities
- Perform occupational therapy evaluations and reassessments in the home setting.
- Develop and implement individualized treatment plans based on patient needs and physician orders.
- Provide therapeutic interventions focused on activities of daily living (ADLs), mobility, strength, coordination, and safety.
- Educate patients, caregivers, and family members on treatment goals and home exercise programs.
- Complete required documentation accurately and in a timely manner.
- Collaborate with physicians, nurses, physical therapists, and interdisciplinary care team members.
- Maintain compliance with Medicare and agency guidelines.
