About The Position

HealthPro Heritage is hiring an Occupational Therapist for a senior living and assisted living facility in Fort Wayne, Indiana. The role is offered as part-time or PRN and involves providing occupational therapy services as part of a caring, collaborative team. Part-time work is approximately 5–6 hours per week, with the possibility of growing into more hours. Compensation is $47–$52 per hour for part-time and $55–$60 per hour for PRN. Requirements

Degree in Occupational Therapy from an accredited institution.
Valid state licensure as an Occupational Therapist, or eligibility to obtain licensure.
Strong clinical assessment and diagnostic skills.
Proficiency in developing and implementing effective treatment plans.
Excellent communication and interpersonal skills with a patient-centered approach.
Ability to lift and assist patients as needed.
Ability to stand for extended periods and perform physical tasks related to therapy.

Responsibilities

Evaluate clients’ physical, emotional, and cognitive abilities using observations and standardized tools.
Develop personalized treatment plans to meet clients’ goals and improve their functional abilities.
Educate clients and their families about improving daily activities, including the use of adaptive equipment and environmental modifications.
Maintain accurate records of evaluations, progress, and treatment plans in compliance with legal and ethical standards.
Advocate for clients’ needs and support access to community resources, services, and accommodations.
