About The Position

FOX Rehabilitation is hiring Occupational Therapists to provide one-on-one care to older adults in Jonesboro, Georgia, and surrounding communities. Clinicians work in private residences and senior living settings, including independent living, assisted living, and memory care, using the Geriatric House Calls™ model. The role focuses on helping patients improve mobility, independence, wellness, longevity, and quality of life. Clinicians may also provide telehealth and other virtual care when appropriate. The position offers autonomy to create, manage, and adjust a treatment schedule, build lasting patient relationships, and work within a collaborative interdisciplinary team. FOX describes its practice as clinician-operated and grounded in evidence-based care, meaningful relationships, and a mission to abolish ageism. Full-time, part-time, and PRN/Fox Flex pathways are available; compensation and benefit eligibility vary by employment type. New graduates may be eligible for the Emerging Professionals Mentor Program.

Requirements

Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Occupational Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Ability to drive to patient homes and senior living communities.
Wear PPE when required.

Responsibilities

Provide one-on-one occupational therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
When applicable, supervise PTA/OTAs, mentor new graduates, or serve as a Clinical Instructor.
