About The Position

We are seeking a compassionate and flexible Occupational Therapist to join an in-house rehabilitation team on a PRN basis. The role provides person-centered care to improve residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes. New graduates are welcome to apply.

Requirements

Active and valid Texas Occupational Therapy license.
Good standing with all regulatory agencies and licensing boards.
Current knowledge of occupational therapy treatment practices.
Ability to manage patients with varying personalities and needs.
Strong communication, collaboration, and clinical decision-making skills.

Nice To Haves

Skilled Nursing Facility (SNF) or long-term care experience preferred.
Experience with the Net Health Electronic Documentation System preferred.

Responsibilities

Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life.
Assess and interpret patient evaluations and test results.
Develop individualized treatment plans using therapeutic techniques to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
Assess patients’ home and living environments and recommend modifications based on their needs and capabilities.
Recommend adaptive equipment and provide training to support independence and safety.
Deliver treatments in accordance with the established plan of care.
Monitor patient responses to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team.
Provide clinical education and support to patients, family members, and caregivers.
Collaborate with interdisciplinary team members to ensure high-quality patient care.
Complete documentation thoroughly and on time in compliance with company policies and state and federal regulations.
Comply with applicable rules, regulations, and procedures.
Attend required meetings as designated by the Director of Rehab.
