About The Position

FOX Rehabilitation is seeking licensed Occupational Therapists to provide one-on-one care to older adults in Lodi, California, and surrounding communities. Care is delivered in patients’ homes and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, longevity, and quality of life through evaluations, individualized plans of care, and evidence-based treatment. Telehealth and other virtual care may also be incorporated when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and collaborate with an interdisciplinary team. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways. New graduates are eligible for the Emerging Professionals Mentor Program. Compensation depends on experience and employment type; the stated regional full-time base pay range is $80,000–$110,000 annually, with opportunities to earn more. Part-time and FOX Flex compensation is prorated.

Requirements

Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Occupational Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including occasional lifting of 25–50 lbs, frequent lifting of 10–25 lbs, and constant lifting of up to 10 lbs.
Ability to perform CPR and provide moderate to maximum assistance with patient transfers and bed mobility.
Ability to reposition patients, stand and walk for extended periods, climb stairs, and perform therapeutic activities.
Ability to drive to patient homes and senior living communities.
Ability to wear PPE when required.

Responsibilities

Provide one-on-one occupational therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Create and manage a treatment schedule and build patient relationships.
Potentially supervise PTA/OTAs, mentor new graduates, or serve as a Clinical Instructor, with additional earning opportunities.
