Occupational Therapist (SIGN ON BONUS) - Summerville, SC
About The Position
FOX Rehabilitation is hiring licensed Occupational Therapists to provide one-on-one care to older adults in Summerville, South Carolina, and surrounding communities. Clinicians treat patients in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. The role focuses on improving mobility, independence, wellness, longevity, and quality of life through individualized, evidence-based care. Clinicians may also incorporate telehealth and other virtual care into their schedules. The position offers autonomy to create and manage a treatment schedule and build lasting patient relationships. FOX describes its practice as clinician-operated, with an interdisciplinary and clinician-driven culture. Full-time, part-time, and PRN/FOX Flex employment pathways are available; benefits depend on role eligibility. New graduates may participate in the Emerging Professionals Mentor Program.
Requirements
- Valid Occupational Therapy license in the state or states of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living communities.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
