About The Position

Concierge Home Care is hiring Home Health Occupational Therapists (OTR). This role offers an opportunity to control your schedule and income, with 1:1 patient care in the home. The coverage area is Fernandina Beach, FL / Nassau County, with territories designed to minimize drive time and cluster scheduling. The position offers a pay-per-visit model with potential for high annual earnings, quarterly bonuses, and mileage reimbursement. Consistent patient volume is provided.

Requirements

• Active, unencumbered Florida Occupational Therapist license
• Valid driver’s license, reliable transportation, and auto insurance
• CPR certification
• Ability to supervise COTAs

Nice To Haves

• Home health experience preferred, but not required

Responsibilities

• Conduct patient evaluations and home environment assessments
• Develop individualized treatment plans based on physician orders
• Provide therapy focused on improving functional mobility and daily living skills
• Educate patients and caregivers on adaptive strategies and safety techniques
• Collaborate with physicians and interdisciplinary care teams
• Supervise and delegate care to Certified Occupational Therapy Assistants (COTAs) when appropriate
