About The Position

FOX Rehabilitation is hiring licensed Occupational Therapists to provide one-on-one care to older adults in Boynton Beach, Florida, and surrounding communities. Clinicians work in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. The role focuses on helping patients improve mobility, independence, wellness, longevity, and quality of life through evaluations, individualized plans of care, and evidence-based treatment. Telehealth and other virtual care may also be incorporated when appropriate.

The position offers autonomy and flexibility, including the ability to create, control, and adjust a treatment schedule and build lasting relationships with patients. Clinicians collaborate with an interdisciplinary team and may educate patients, caregivers, and senior living staff. FOX lists full-time, part-time, and PRN / FOX Flex employment pathways; benefits and compensation depend on eligibility and employment type. New graduates may be eligible for the Emerging Professionals Mentor Program.

Requirements

• Valid Occupational Therapy license in the state or states of practice, or eligibility to obtain one.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
• Ability to wear PPE when required.

Responsibilities

• Provide one-on-one occupational therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
