Responsibilities

Conduct comprehensive occupational therapy evaluations for residents and develop individualized treatment plans to improve activities of daily living, mobility, and functional independence.
Implement evidence-based interventions, monitor progress, and adjust plans to maximize functional outcomes.
Collaborate with the interprofessional team including nursing, speech therapy, physical therapy, social work, and case management to ensure coordinated care.
Document evaluation findings, treatment progress, and discharge recommendations in the resident’s medical record; communicate with families and care teams as appropriate.
Participate in discharge planning and provide caregiver education to support ongoing rehabilitation in the home or care setting.
