About The Position
Family Tree Rehabilitation, part of The Campus of Anna Maria, is seeking an Occupational Therapist Registered (OTR) to support its therapy team at a family-owned and operated skilled nursing facility in Aurora, Ohio. The facility provides high-quality, compassionate, individualized rehabilitation services in a peaceful, wooded campus setting. The role evaluates residents and develops, implements, and updates individualized occupational therapy plans of care to help them achieve maximum independence and quality of life. Care may be provided in a skilled nursing facility or home health care setting. The position offers full-time and flexible opportunities, with hourly pay of $39–$47 based on experience and qualifications.
Requirements
- Current Ohio Occupational Therapist (OTR) license in good standing.
- Graduate of an accredited Occupational Therapy program.
- Strong clinical judgment, communication, and time-management skills.
- Compassionate, resident-centered approach.

- Skilled nursing or rehabilitation experience.
Responsibilities
- Perform occupational therapy evaluations and establish individualized plans of care.
- Provide skilled occupational therapy treatments in accordance with physician orders.
- Supervise and collaborate with Certified Occupational Therapy Assistants (COTAs), as applicable.
- Accurately document evaluations, treatments, resident progress, and discharge plans.
- Participate in interdisciplinary care planning and team communication.
- Ensure compliance with facility policies, professional standards, and state and federal regulations.
