About The Position

FOX Rehabilitation is hiring licensed Occupational Therapists to provide one-on-one care to older adults in their homes and in Senior Living Communities in Rochester Hills, Michigan, and surrounding communities. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care may also include telehealth and other virtual services when appropriate. The role offers autonomy to create and manage a treatment schedule, build ongoing patient relationships, and work in a collaborative, interdisciplinary community-based practice. FOX lists full-time, part-time, and PRN / FOX Flex employment pathways. Full-time positions are salaried with benefits; part-time positions are salaried with benefits eligibility based on minimum hours requirements; PRN / FOX Flex positions are compensated per unit. New graduates may be eligible for the Emerging Professionals Mentor Program. Compensation varies based on experience and employment type. Eligible roles may include medical, dental, and vision insurance, a 401(k) with company match, unlimited continuing education access, professional certification reimbursement, and clinical and nonclinical career advancement pathways. Clinicians may have opportunities to earn additional compensation through referrals, clinical instruction, mentoring, supervising assistants, work above a bi-weekly quota, and creating written or social content.

Requirements

Hold a valid Occupational Therapy license in the state(s) of practice or be eligible to obtain one.
Have a degree from an accredited Occupational Therapy program.
Have basic computer and EMR proficiency.
Hold current CPR certification.
Be able to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Be able to drive to patient homes and senior living communities.
Wear PPE when required.

Responsibilities

Provide one-on-one occupational therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Provide therapy to support patient wellness, longevity, mobility, and functional independence.
Document care in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
Use professional development and additional earning opportunities as applicable, including clinical instruction, mentoring, and supervising PTA/OTAs.
