About The Position

Victorian Post Acute is a beautiful 90-bed, rehabilitation-focused skilled nursing facility dedicated to providing outstanding care for both short-term rehabilitation and long-term care residents. We are currently seeking a compassionate and motivated Occupational Therapist (OTR) to join our growing in-house therapy team. Requirements

Current California Occupational Therapist (OTR) license in good standing.
Strong communication, organizational, and interdisciplinary collaboration skills.
Passion for delivering resident-centered care.

Responsibilities

Evaluate residents and develop individualized occupational therapy treatment plans.
Provide skilled occupational therapy services in accordance with the established plan of care.
Supervise Certified Occupational Therapy Assistants (COTAs) in compliance with California practice guidelines.
Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families to achieve optimal outcomes.
Participate in care conferences, rehabilitation meetings, and discharge planning.
Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
Educate residents, families, and caregivers on adaptive equipment, home programs, safety strategies, and functional independence techniques.
Recommend and coordinate appropriate durable medical equipment (DME) to enhance resident safety and independence.
Maintain a safe, organized therapy environment and promptly report equipment concerns.
