About The Position

The Occupational Therapist / Physiotherapist is an integral member of the Aging Well Clinic’s interprofessional clinical care team. Working both in-clinic and through community outreach, the clinician provides comprehensive assessment, consultation, counselling, and short-term management for complex older adults. This role works in close collaboration with the clinic’s Geriatricians and Nurse Practitioners and develops strong partnerships with community providers to support coordinated, patient- and family-centred care.

Requirements

  • Registered and in good standing with the respective regulatory body: College of Occupational Therapists of Ontario (COTO) or College of Physiotherapists of Ontario
  • Master’s degree in Occupational Therapy or Physical Therapy (or equivalent) from a recognized university
  • Minimum 2–3 years experience working with the geriatric population
  • BCLS certification required, or willingness to obtain
  • MOCA certification required, or willingness to obtain within 3 months of hire
  • Valid Ontario Driver’s License, access to a vehicle, and a clean driving abstract
  • Excellence in interpersonal communication and team building, fostering positive collaboration with internal and external partners
  • Advanced assessment and management skills across physical and cognitive domains
  • Working knowledge of: Geriatric syndromes and geriatric psychiatry
  • Working knowledge of: Community resources and crisis intervention
  • Working knowledge of: Regulated Health Professions Act, Mental Health Act, Health Care Consent Act, and Substitute Decisions Act
  • Demonstrated commitment to evidence-based, patient- and family-inspired care
  • Superior verbal and written communication skills
  • Strong critical thinking, problem-solving, and clinical judgment
  • Ability to work effectively both independently and within an interprofessional team
  • Commitment to clinical innovation and lifelong learning
