PHMC serves as both a direct service provider to individuals, families, and communities across the region and as an intermediary agent — managing large-scale contracts, government and philanthropic partnerships, and multidisciplinary initiatives that require operational sophistication, strategic leadership, and deep mission alignment. The Nursing Supervisor – Medical Respite will supervise nursing staff supporting clinical services in post-acute medical care to individuals released from a hospital stay following an illness, surgery, or an injury, and are too ill or frail to recover in a shelter-based setting. Participants are provided with respite-based services, including medication management, discharge planning, appointment transportation and scheduling, case coordination, and connecting clients to primary care. This individual will work closely with the Medical Director, Respite Program Administrator, and other respite staff to support daily respite operations, working relationships with referral sources, on-site patient evaluations, supporting clinical care and staffing in all respects, and secondary support with other administrative matters like IT and food service. All services are provided in collaboration with on-site or community-based case managers and with any other healthcare for the homeless (HCH) staff providing services to the target population. The Nursing Supervisor – Medical Respite will report to the Director of Nursing and serve on PHMC Health’s nursing leadership team, along with the Nursing Supervisors for Shelter Nursing and Health Centers.
