About The Position

Our Lady of Consolation, a proud member of Catholic Health, is a recognized leader in providing exceptional quality driven care, offering Long Term, Post-Acute and Skilled Nursing services. Located on the beautiful Great South Bay, the facility is a 345-bed nursing and rehabilitative care center. The organization's nurses, physicians, and support staff are dedicated to providing patient-driven health care in a compassionate environment, aiming to create a culture of healing and safety while upholding its religious mission. The Nursing Care Coordinator is responsible for coordinating all direct nursing services to patients/residents in collaboration with the unit nursing staff. This role also functions as a geriatric nursing specialist and a resource to the staff on the units and to other institutional departments as necessary.

Requirements

  • Graduate of an accredited School of Nursing Program or an accredited College RN degree
  • Minimum of three (3) years of experience in long term care or a combination of long-term care and acute care
  • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to long term care
  • Licensed to practice as a Registered Nurse in New York State

Responsibilities
