About The Position

We are seeking an experienced Nurse Supervisor – Care Management to provide operational and clinical leadership within an acute care hospital setting. This role oversees utilization review, discharge planning, care coordination, and case management operations while ensuring regulatory compliance, quality outcomes, and efficient patient throughput. The ideal candidate will possess strong leadership abilities, extensive case management expertise, and a comprehensive understanding of utilization management and discharge planning processes.

Requirements

  • Active New York State Registered Nurse (RN) License.
  • Minimum 5 years of clinical nursing experience.
  • Minimum 2 years of experience in discharge planning and utilization review.
  • Certified Case Manager (CCM) Certification – Required.
  • PRI Certification – Required.
  • Proven leadership or supervisory experience in an acute care hospital setting.
  • Strong communication, critical-thinking, and organizational skills.

Nice To Haves

  • Previous Nurse Supervisor or Care Management leadership experience.
  • Experience within large academic medical centers or public hospital systems.
  • Knowledge of Medicare, Medicaid, managed care, and utilization management regulations.
  • Experience with EPIC and case management software platforms.

Responsibilities

  • Provide leadership, supervision, and guidance to the Care Management team.
  • Oversee utilization review, discharge planning, and case management operations.
  • Support complex case reviews and escalation management decisions.
  • Ensure compliance with hospital policies, regulatory requirements, and quality standards.
  • Collaborate with physicians, nursing leadership, social workers, and interdisciplinary teams.
  • Monitor departmental performance metrics, productivity, and workflow efficiency.
  • Evaluate care management practices to improve patient outcomes and resource utilization.
  • Ensure accurate documentation, reporting, and regulatory compliance.
  • Participate in quality improvement initiatives and departmental planning.
  • Assist with staff development, coaching, performance evaluations, and competency management.
