About The Position

Navitas Healthcare, LLC is seeking a RN - Care Manager for an exciting Contract job in Bronx, NY. This role involves coordinating patient care throughout their inpatient stay, conducting utilization reviews, and supporting appropriate level-of-care decisions. The position requires developing and implementing comprehensive discharge and transition-of-care plans, collaborating with a multidisciplinary team, and ensuring timely interventions and safe discharge planning. Accurate documentation, compliance with regulations, and proficiency with EMR systems are essential.

Requirements

Bachelor of Science in Nursing (BSN).
Active New York State Registered Nurse (RN) License.
Minimum 5 years of clinical RN experience.
Prior experience in Care Coordination, Utilization Review, and Discharge Planning.
PRI & Screen Certification (required or willingness to obtain if applicable).
Current Basic Life Support (BLS) Certification.
Strong proficiency with EMR systems and computer applications.
Excellent communication, critical thinking, and organizational skills.

Nice To Haves

Experience working in an acute care hospital setting.
Strong interdisciplinary care coordination experience.
Knowledge of payer guidelines and utilization management processes.
Experience with discharge planning and transition-of-care programs.

Responsibilities
