About The Position

Matrix Medical Network is seeking a PRN Nurse Practitioner to conduct in-home and facility-based Comprehensive Health Assessments for adult and geriatric members. The assessments offer insight into overall health and may help improve care quality and address multiple care gaps in a single visit. Providers conduct assessments at members’ residences, including private homes and Skilled Nursing Facilities. Visits are scheduled according to provider availability, with assessments conducted Sunday through Saturday from 8 a.m. to 7 p.m. Compensation is based on completed assessments; providers averaging 20–100 visits per month earn $1,800–$11,500 per month, at a stated rate of $90–$115 per assessment depending on state of residence. The role includes onboarding, a preceptor, individual feedback, and access to continuing education. Matrix may support additional state licensure and credentialing when business needs exist. PRN providers may transition to full-time opportunities and receive a full-time sign-on bonus.
