About The Position
Heartline Staffing is hiring a Nurse Practitioner in Warwick, Rhode Island. This role focuses on providing equitable, effective value-based healthcare to local Medicare patient populations. You will be supported by a large care team, allowing you to concentrate on quality of care rather than volume. The value-based model and bonuses are structured to reward outcomes, low hospital admissions, preventive medicine, and an outstanding patient experience. Clinics are collaborative, typically with 3-5 providers, including a mix of Nurse Practitioners and Primary Care Physicians.
Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate REQUIRED
- National certification as a Family, Adult-Gerontology Primary Care, Adult, or Gerontological Nurse Practitioner REQUIRED
- Active, non-probationary Rhode Island Nurse Practitioner license REQUIRED
- Active DEA license REQUIRED
Nice To Haves
- Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs) preferred
Responsibilities
- Provide value-based primary care to Medicare patients
- Deliver preventive medicine and work to reduce hospital admissions
- Manage ongoing care for patients, including frequent follow-up for complex patients
- Collaborate with physicians, fellow Nurse Practitioners, and the care team
- Document accurately to support quality, coding, and risk assessment
