Nurse Practitioner | Primary Care | $120K–$140K + Bonus
About The Position
Heartline Staffing is recruiting a Nurse Practitioner for a full-time primary care opportunity in Peoria, Illinois. This position provides value-based healthcare to local Medicare patient populations and is supported by a multidisciplinary care team so providers can focus on quality of care, preventive medicine, and patient outcomes. The Nurse Practitioner will provide primary care within a value-based healthcare model focused on preventive medicine, patient outcomes, and reducing avoidable hospital admissions. Providers are supported by an established care team that includes a scribe, Medical Assistant, and Social Worker.
Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program graduate REQUIRED
- National Nurse Practitioner certification in an accepted specialty REQUIRED
- Active, non-probationary Nurse Practitioner license appropriate for practice in Illinois REQUIRED
- Active DEA license REQUIRED
- Family Nurse Practitioner certification
- Adult-Gerontology Primary Care Nurse Practitioner certification
- Adult Nurse Practitioner certification
- Gerontological Nurse Practitioner certification
Nice To Haves
- Understanding of HCC documentation
- ICD-10 coding knowledge
- Experience with Health Risk Assessments (HRAs)
Responsibilities
- Provide primary care within a value-based healthcare model focused on preventive medicine, patient outcomes, and reducing avoidable hospital admissions.
- Focus on quality of care, preventive medicine, and patient outcomes.
- See approximately 13-16 patients per day.
- Conduct approximately 20-minute regular visits.
- Conduct approximately 40-minute new-patient and full diagnostic visits.
- See patients approximately every 3 months, with more frequent visits for complex cases as needed.
