About The Position

Circadia Health is hiring a per diem Nurse Practitioner to provide on-site care and support the integration of its AI-powered health monitoring platform into senior-care operations. The role combines direct patient care with hands-on training and coaching for facility teams, using predictive health signals to support earlier interventions and reduce preventable readmissions.

The Nurse Practitioner will conduct visits at skilled nursing facilities and patient homes within a 30–40 mile radius of Marlborough, Massachusetts. Visits focus on new admissions, facility-flagged residents, and patients identified through Circadia alerts. The role targets 25 or more visits per day where appropriate and requires same-day documentation or completion within 24 hours. For locations outside the stated radius, mileage and lodging are covered according to company policy.

The position is per diem, with a minimum commitment of 4–8 days per month and the opportunity to work additional days based on patient demand. Compensation is paid per visit, with a listed range of $3,000–$5,200 per month. Circadia provides AI documentation tools, in-app training, integrated workflows, a company laptop, mobile tools, and travel reimbursement at the federal rate.

Requirements

• Certified and licensed Nurse Practitioner in Massachusetts.
• ANCC certification, or equivalent, in Adult, Family, Geriatric, or Acute care.
• At least 2 years of Nurse Practitioner experience with complex adult populations.
• Ability to practice independently.
• Eligible for Medicare participation.
• Valid Massachusetts driver’s license and comfort with local travel.
• Ability to thrive in a dynamic, evolving startup environment.
• English proficiency.

Nice To Haves

• Familiarity with Google Suite, multiple EMRs, Slack, and AI transcription or other AI tools.
• Spanish language proficiency.

Responsibilities

• Conduct initial and follow-up assessments of flagged residents.
• Initiate or adjust plans of care, complete consent, and coordinate with facility providers.
• Train and coach staff at the point of care and standardize Circadia use within clinical workflows.
• Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Complete encounter documentation and charts within the same day, and use AI-powered tools to complete notes within 24 hours.
• Efficiently round across designated skilled nursing facilities, typically completing 25 or more visits per day where appropriate.
• Focus visits on new admissions, facility-flagged patients, and Circadia alerts.
• Share field insights to improve workflows and technology integration in clinical settings.
• Support Nurse Practitioner onboarding, share best practices, and collaborate with clinical operations and product teams.
• Cover skilled nursing facility sites and patient homes within a 30–40 mile radius.
