Per Diem Nurse Practitioner
About The Position
Circadia Health is hiring an on-site Nurse Practitioner in Marlborough, Massachusetts, to provide proactive care to residents in skilled nursing facilities and patient homes. The role combines direct patient care with use of Circadia’s AI-enabled health platform to identify and respond to predictive health signals, support earlier interventions, and reduce avoidable readmissions. The Nurse Practitioner will assess patients, act on clinical insights, and help facility teams incorporate Circadia into care workflows through training, coaching, demonstrations, and shadowed assessments. The position involves high-volume rounding across designated facilities, generally targeting 25 or more visits per day when appropriate, with a minimum commitment of 4–8 days per month and the opportunity to work additional days based on patient demand. The role covers SNF sites and patient homes within a 30–40 mile radius; mileage and lodging for locations beyond that radius are covered according to company policy. Compensation is paid per visit.
Requirements
- Certified and licensed Nurse Practitioner in Massachusetts, with ANCC certification or equivalent in Adult, Family, Geriatric, or Acute care.
- Eligible for Medicare participation; the company bills on the Nurse Practitioner’s behalf.
- At least 2 years of Nurse Practitioner experience with complex adult populations.
- Ability to practice independently.
- Valid Massachusetts driver’s license and comfort with local travel.
- Ability to thrive in a dynamic, evolving startup environment.
- English language proficiency.

- Technology proficiency, including Google Suite, multiple EMRs, Slack, and AI transcription or other AI tools.
- Spanish language proficiency.
Responsibilities
- Conduct initial and follow-up assessments of flagged residents.
- Initiate or adjust plans of care, complete consent, and coordinate with facility providers.
- Train and coach staff at the point of care and standardize the use of Circadia in clinical workflows.
- Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent patient decline.
- Complete documentation and charts on the same day, with all notes completed within 24 hours of each encounter using AI-powered tools.
- Complete high-volume visits across designated skilled nursing facilities, focusing on new admissions, facility-flagged patients, and Circadia alerts.
- Provide field insights to improve workflows and technology integration in clinical settings.
- Support Nurse Practitioner onboarding, share best practices, and collaborate with clinical operations and product teams.
- Travel to skilled nursing facilities and patient homes within the assigned region.
