Nurse Practitioner - 3 Days - South County/Tesson Ferry
About The Position
Esse Health is seeking a Nurse Practitioner for its Primary Care Partners office at 13303 Tesson Ferry. This part-time outpatient role provides comprehensive primary care to adult patients in a collaborative, physician-led practice. The schedule is three days per week, Wednesday through Friday, from 8:00 a.m. to 5:00 p.m., for 24 hours per week. The position has no nights, weekends, or holidays and is not benefit eligible unless applicable minimum-hours requirements are met. The Nurse Practitioner will build one-to-one relationships with patients and clinical staff, work independently while collaborating with a physician partner, and contribute to coordinated, patient-centered care.
Requirements
- Master’s Degree of Science in Nursing.
- Current Nurse Practitioner licensure in Missouri, or eligibility.
- Certified Nurse Practitioner, or eligibility.
- Strong clinical, communication, and interpersonal skills.
- Commitment to patient-centered, team-based care.
- Ability to work independently while collaborating closely with physicians.
Responsibilities
- Develop one-to-one relationships with patients and clinical staff.
- Provide comprehensive primary care services to adult patients.
- Conduct patient evaluations, physical exams, and diagnostic assessments.
- Diagnose and manage acute and chronic conditions.
- Develop and implement treatment plans in collaboration with a physician partner.
- Emphasize preventive care, wellness, and patient education.
- Document patient care accurately and efficiently in the NextGen electronic health record.
- Coordinate care with specialists and other members of the healthcare team.
- Participate in quality improvement and care coordination initiatives.
