This position involves conducting one-time disability evaluations for veterans on-site at an established clinic in Vernal, UT. The role does not involve ongoing patient care, treatment, prescriptions, or management. Clinic hours are from 8:00 AM to 6:00 PM. Candidates must reside within a daily commute range, specifically within 50 miles of Vernal, UT.
Requirements
- Active, unrestricted Nurse Practitioner license in the state where you will work
- Board certification
- NPI number
- Current ACLS and BLS
- Active DEA
- A home address within 50 miles of Vernal, UT
- CV in month and year format
Responsibilities
- Examine veterans and record findings on standard disability evaluation forms.
- Conduct one-time evaluations, not ongoing care.
- Work out of an established clinic in Vernal, UT and see veterans on site.
