About The Position

Integrated Wound Care is seeking a compassionate and skilled Nurse Practitioner for a part-time opportunity focused on wound care. The company provides wound care services to skilled nursing, long-term care, and rehabilitation facilities, aiming to exceed clinical standards while delivering care efficiently. Its programs help facilities reduce wound-related hospitalizations and improve outcomes for residents and patients. In this role, the Nurse Practitioner will contribute to patient care through wound assessment, treatment planning, education, and progress monitoring. The position is located in Portsmouth, New Hampshire.

Requirements

Hold an active license in New Hampshire or be willing to obtain one.
Have a minimum of one year of wound care experience.
Be able to work independently and as part of a team.
Have critical-thinking and decision-making skills.
Have excellent verbal and written communication skills.
Have strong time-management and organizational abilities.
Be comfortable with technology and electronic medical records.

Responsibilities

Conduct wound assessments and provide personalized treatment plans for patients.
Work collaboratively with healthcare professionals to ensure optimal patient care.
Complete documentation accurately and in a timely manner.
Educate patients and their families on wound care, prevention, and management.
