Matrix Medical Network•Newark, NJ
•Onsite

About The Position

Matrix Medical Network is hiring a PRN Nurse Practitioner to conduct in-home and Skilled Nursing Facility Comprehensive Health Assessments for adults and geriatric members. During visits lasting up to one hour, providers review and observe members’ current health, medical history, medication adherence, social environment, and other health risks. These assessments help improve quality of care and may close multiple care gaps in a single visit. Providers conduct assessments, administer or order point-of-care and lab-based testing as needed, document according to Matrix guidelines and protocols, and coordinate with physicians, case managers, social workers, family members, caregivers, and other medical personnel. The role offers scheduling flexibility, with assessments conducted Sunday through Saturday from 8 a.m. to 7 p.m. Compensation is based on completed assessments; providers averaging 20–100 visits per month earn $1,800–$11,500 monthly, based on a per-assessment rate of $90–$115 depending on state of residence. Matrix provides onboarding, a preceptor, individual feedback, access to continuing education, and may support additional state licensure and credentialing when business needs exist. PRN providers may transition to full-time opportunities.
