About The Position
Prime Healthcare Illinois Medical Center is seeking an experienced, dedicated, and compassionate Nurse Practitioner to join its Primary Care team in an established outpatient clinic in Chicago, Illinois. The role provides comprehensive, patient-centered primary care, including preventive care, chronic disease management, and acute visits. The Nurse Practitioner manages an individual patient panel and works closely with physicians, advanced practice providers, and clinical support staff to support continuity of care. This is a full-time, 40-hour-per-week position with a day shift and minimal call responsibility. Requirements
- Licensed and certified as a Nurse Practitioner in the state of Illinois.
- Board-certified in Family Medicine, Adult-Gerontology, or a related field.
- Strong clinical assessment, communication, and documentation skills.
- Commitment to providing high-quality, compassionate care.
Nice To Haves
- Prior experience in primary care is preferred but not required.
- Ukrainian proficiency is preferred.
- Experience working in an outpatient setting.
