About The Position

The company develops and provides wound care services to skilled nursing, long-term care, and rehabilitation facilities, aiming to exceed clinical standards in wound healing while managing costs and service delivery. Its programs help facilities reduce wound-related hospitalizations and improve outcomes for residents and patients. In this role, the Nurse Practitioner will contribute to patient care and outcomes through wound assessment, treatment, education, and progress monitoring.

Requirements

Active license in the state of employment.
At least 1 year of wound care experience.
Ability to work independently and as part of a team.
Exceptional critical-thinking and decision-making skills.
Excellent verbal and written communication skills.
Strong time-management and organizational abilities.
Comfort with technology and electronic medical records.

Responsibilities

Conduct wound assessments and provide personalized treatment plans for patients.
