About The Position

WellBe Senior Medical is hiring a Nurse Practitioner to deliver value-based, field-based primary care directly in patients’ homes in Findlay, Ohio. The role focuses on whole-person care, patient autonomy, and supporting high-risk seniors through an interdisciplinary care team. The position offers a $10,000 sign-on bonus and a salary of $104,763–$166,750, plus mileage and full benefits.

The Nurse Practitioner conducts comprehensive primary care visits and physical exams in patients’ homes, develops longitudinal care plans for chronic diseases, addresses acute needs to help prevent emergency-room visits, and coordinates safe care transitions with the interdisciplinary team. Support includes daily team check-ins, ongoing training, and one-to-one mentorship.

Applicants must hold an active, unencumbered Nurse Practitioner license and board certification through ANCC or AANP, and have a valid driver’s license and vehicle for local field-based travel. A master’s degree is required; a focus in Adult, Geriatric, Acute Care, and/or Family practice is preferred. Employment is contingent upon required screenings and checks. WellBe does not offer employment-based visa sponsorship.

Requirements

• Master’s degree as a Nurse Practitioner.
• Active, unencumbered Nurse Practitioner license.
• Board certification through ANCC or AANP.
• Valid driver’s license and vehicle for local field-based travel.
• Legally authorized to work in the United States without employer sponsorship now or in the future.
• Successfully complete a pre-employment drug screening and required background checks; continued employment is contingent on satisfactory applicable checks and compliance with the Drug-Free Workplace Policy.
