About The Position

WellBe Senior Medical is hiring a Nurse Practitioner (APP) to provide value-based care directly in patients’ homes in Findlay, Ohio. The role centers on autonomous, whole-person primary care for seniors, with attention to social determinants of health and support from an interdisciplinary care team. The position offers a $10,000 sign-on bonus, a salary of $104,763–$166,750, mileage, and full benefits. Team support includes daily check-ins, ongoing training, and one-to-one mentorship.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license.
Board certification through ANCC or AANP.
Valid driver’s license and a vehicle for local field-based travel.
Must be legally authorized to work in the United States without current or future employer visa sponsorship.
Must successfully complete pre-employment drug screening and meet applicable drug-free workplace testing requirements.
Employment is contingent upon successful background checks, including monthly FACIS checks.

Responsibilities

Conduct comprehensive primary care visits and physical exams in patients’ homes.
