About The Position

ChenMed is seeking a Spanish-speaking Nurse Practitioner to provide primary care to older adults in a physician-led, mission-driven organization focused on affordable, value-based care for seniors. The Nurse Practitioner serves as a primary care provider for a panel of up to 450 patients and is accountable for clinical outcomes, cost-effectiveness, and building trusting relationships that support patients in changing health behaviors and reaching health goals. The provider independently manages acute and chronic illnesses and remains accountable for each patient’s care and outcomes, consulting specialists when appropriate while staying involved in detailed care. The role includes assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs. The Nurse Practitioner leads a care team and collaborates with hospitalists, transitional-care teams, managers, medical directors, and other healthcare professionals. They also contribute to center management, provider coverage, recruiting, quality and risk programs, and a positive organizational culture. The position is outcomes-focused. Providers work toward goals involving patient admissions, appointment capacity, patient experience, clinical gap closure, and medical costs. They are expected to make themselves accessible to patients, follow evidence-based medicine, continually develop leadership skills, and help improve care quality and center operations. The role requires compliance with departmental objectives, performance standards, regulations, quality requirements, and company policies and procedures. The posting identifies Spanish speaking as part of the role title; it separately states that English fluency is required and that other-language requirements may be specified for particular centers.
