About The Position

The Nurse Practitioner will support the Transition of Care/ Medication Refill Team with refilling medications for patients without an assigned PCP to decrease hospital admissions and emergency room visits. Will also provide precepting by telehealth and scheduling in person/telehealth visits for those patients who need to have follow up care. The Nurse Practitioner also will provide comprehensive, cost-effective health care to families which includes assessment, diagnosis, and treatment according to the standards of advanced nursing practice.

Requirements

• Graduate of an ANA accredited Nurse Practitioner program.
• Master’s degree in nursing required.
• Minimum of 2 years of experience.
• Nationally certified as a Family Nurse Practitioner.
• Massachusetts licensed.
• Current DEA/Mass Controlled Substance license for prescriptive authority.
• BLS required.
