Nurse Practitioner- Fee for Service

About The Position

TruHealth is seeking an advanced practice provider—a nurse practitioner or physician assistant—with primary care or internal medicine experience, preferably with a focus on geriatrics. Providers primarily work in nursing homes or assisted living communities, collaborating with providers, case managers, facility partners, and health plan leadership to improve outcomes and quality of life for long-term care residents. The role involves providing personalized, coordinated care that manages acute and chronic conditions, addresses barriers to care, and helps prevent rehospitalization. Providers exercise independent judgment and consult with a supervising physician as appropriate. The position also involves implementing Treat in Place Protocols in compliance with federal and state regulations. This is a fee-for-service position.

Requirements

Current APRN or PA license.
Degree from an accredited APRN or PA program.
Three years of experience in clinical nursing or rehabilitation in geriatric populations and/or fee-for-service experience.
Experience with electronic health records.
Working knowledge of Microsoft Word, Outlook, and Excel.

Responsibilities

Assess patients’ medical and mental health needs and provide plans of care that proactively manage conditions and address barriers to care.
Ensure compliance with local, state, and federal requirements related to clinical services provided.
Prescribe medications and order laboratory work, diagnostic procedures, and consultations.
Monitor patients’ compliance with and response to treatment, and modify treatment plans as needed.
Collaborate with providers, case managers, facility partners, and health plan leadership to improve patient outcomes and implement Treat in Place Protocols.
May provide training and advice to facility staff.
