About The Position
This position is for a Nurse Practitioner in the Heart Success program at Atlantic Health Morristown Medical Center. The role involves conducting patient evaluations, diagnosing and managing cardiovascular conditions, ordering and interpreting tests, developing treatment plans, and educating patients and families. The Nurse Practitioner will collaborate with physicians and the multidisciplinary team to optimize patient outcomes and maintain accurate documentation. Requirements
- Heart Failure or Advanced Heart Failure experience
- Cardiology or Cardiovascular Medicine experience
- Critical Care experience
- Electrophysiology experience
- Cardiothoracic Surgery experience
Responsibilities
- Conduct patient evaluations, physical exams, and follow-up visits.
- Diagnose and manage acute and chronic cardiovascular conditions.
- Order and interpret diagnostic tests and imaging.
- Develop treatment plans, prescribe medications, and coordinate care.
- Educate patients and families on disease management and treatment plans.
- Collaborate with physicians and the multidisciplinary team to optimize outcomes.
- Maintain accurate and timely documentation.
