About The Position
PruittHealth Hospice is seeking a Nurse Practitioner to provide hospice and palliative care to patients in their homes or care facilities. The role involves collaborating with the hospice care team, assessing adult and geriatric patients, evaluating and managing plans of care, maintaining continuity of care, and completing documentation accurately and promptly. PRN or part-time work is available. Requirements
- Hold current, active, unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in the state of practice.
- Be board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.
Responsibilities
- Serve as a proactive, collaborative team member in a hospice care environment.
- Conduct comprehensive adult and geriatric assessments of hospice patients and evaluate and manage their plans of care.
- Demonstrate commitment to caring for patients and partners.
- Complete documentation and records in a timely and accurate manner.
- Follow established policies and procedures related to the scope of practice.
- Maintain continuity of patient care in a hospice setting.
