About The Position
PruittHealth Hospice is hiring a full-time Nurse Practitioner to provide hospice care to patients in their homes or care facilities. The role involves working as part of a specialized hospice-care team and conducting comprehensive adult and geriatric patient assessments, evaluating patients, and managing plans of care. The Nurse Practitioner is expected to support continuity of patient care, maintain accurate and timely documentation, and follow established policies and procedures within the scope of practice. Requirements
- Hold current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in the state of practice.
- Be board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.
Responsibilities
- Work collaboratively as a proactive team member in a hospice-care environment.
- Conduct comprehensive assessments of adult and geriatric hospice patients.
- Evaluate and manage patients’ plans of care.
- Demonstrate commitment to caring for patients and partners.
- Complete documentation and records accurately and promptly.
- Follow established policies and procedures within the scope of practice.
- Maintain continuity of patient care in a hospice setting.
