Hospice & Palliative Care Nurse Practitioner (NP)- PRN

About The Position
The Nurse Practitioner rounds in hospitals and skilled nursing facilities, reviews patients receiving services, and updates care plans. Under the direction of the Medical Director, the Nurse Practitioner assesses patients, reviews medications, creates appropriate care plans, and provides and evaluates care. The role also involves assisting with advance directives and discussing goals of care, disease process and progression, and grief with patients and caregivers. The Nurse Practitioner works with the Director of Patient Care Services and Executive Director to provide regular education about palliative and hospice care to the community and agency staff.
Requirements
- Current license from the state Board of Registered Nursing.
- Master’s degree from an accredited Nurse Practitioner program.
- National certification as a Nurse Practitioner or eligibility for certification; certification must be obtained within the first six months of employment.
- Valid driver’s license and reliable transportation.
- Able to pass a criminal background check and national sex offender clearance.
- Must be able to perform patient care as needed.
Nice To Haves
- A minimum of one year of Nurse Practitioner experience is preferred.
- Previous hospice experience is preferred.
Responsibilities
