About The Position

Requirements
- Hold current, active, unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licenses in the state of practice.
- Be board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.
Responsibilities
- Work proactively and collaboratively as a member of a hospice care team.
- Conduct comprehensive adult and geriatric assessments of hospice patients.
- Evaluate and manage patients’ plans of care.
- Demonstrate commitment to caring for patients and partners.
- Complete documentation and records accurately and in a timely manner.
- Follow established policies and procedures related to scope of practice.
- Maintain continuity of patient care in a hospice setting.
