Nurse Practitioner/Physician Assistant
About The Position
Chestnut Health is a high-growth physician group specializing in physical medicine and rehabilitation (PM&R). It provides care to patients in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs), partnering with medical teams to deliver comprehensive, patient-centered care focused on restoring function, managing pain, and improving mobility. The Nurse Practitioner/Physician Assistant will work part-time or on a 1099 contract basis, with 8–10 hours available per week and a flexible schedule. The role involves assessing patients, collaborating on treatment plans, and maintaining documentation in electronic medical records.
Requirements
- Active Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of practice.
- For Nurse Practitioners: ANCC or AANP certification.
- Current DEA and state Controlled Substance License, required upon start date.

- Experience in a skilled nursing facility (SNF), pain management, or physiatry.
- Interest in physical medicine and rehabilitation (PM&R).
Responsibilities
- Conduct patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
- Collaborate with therapists and the facility’s medical team on treatment plans.
- Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
