About The Position
Ennoble Care is seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and palliative care patients in the Philadelphia, Pennsylvania region on a full-time basis. The provider will apply medical techniques and principles under physician supervision and care for patients with chronic diseases. Ennoble Care is a mobile primary care, palliative care, and hospice provider whose clinicians visit patients at home and offer programs including remote patient monitoring, behavioral health management, and chronic care management.
Requirements
- Board certified, with a valid Nurse Practitioner or Physician Assistant certification in Pennsylvania.
- Strong clinical skills and the ability to confidently and accurately treat, assess, and diagnose patients.
- Positive demeanor and strong interpersonal skills.
- Ability to problem-solve confidently and work independently.
- One to two years of primary care experience with geriatric patients.
Nice To Haves
- Hospice experience is highly preferred but not required.
Responsibilities
- Provide high-quality care to primary and palliative care patients in and around Philadelphia, Pennsylvania.
- Assess patients’ health through physical examinations and by obtaining, updating, and reviewing medical histories; develop appropriate care plans.
- Communicate effectively with office staff and clinical operations through internal platforms.
- Provide patient-centered care to a panel of patients.
- Develop clinical impressions, diagnoses, and treatment plans.
- Build trust and consult with patients, families, and caregivers during visits.
- Order diagnostic testing as medically necessary.
- Prescribe medications, supplies, and appropriate medically necessary interventions.
