Nurse Practitioner / Physician Assistant - Part Time
About The Position
Impact Health PC is an advanced practitioner group serving skilled nursing facilities across the Northeast, specializing in Medical, Psychiatric, Wound Care, and Palliative Care. The organization offers practitioners autonomy, flexible scheduling, and clinical support. The part-time Nurse Practitioner will provide medical and primary care to residents in skilled nursing facilities in Shelton, Connecticut. The role includes assessing and managing acute and chronic conditions, developing treatment plans, and collaborating with physicians, facility staff, and interdisciplinary care teams. Workdays are scheduled Monday through Friday during normal business hours, with no nights, weekends, or holidays. Travel may be up to 25%.
Requirements
- Active Nurse Practitioner (NP) license in Connecticut.
- Strong clinical assessment and communication skills.
- Ability to work independently and collaboratively with a clinical team.
- Proficiency with EHRs and clinical technology.
Nice To Haves
- Two or more years of clinical experience preferred.
- Experience in skilled nursing facilities, primary care, hospitalist medicine, or acute care preferred.
Responsibilities
- Assess, diagnose, and manage acute and chronic medical conditions.
- Develop individualized treatment plans and provide ongoing patient care.
- Complete timely clinical documentation and place appropriate orders.
- Collaborate with physicians, nursing staff, and interdisciplinary care teams.
- Maintain compliance with facility, state, and federal regulations.
