About The Position

Essentia Health's Fosston clinic is seeking an experienced Advanced Practice Clinician to join their Family Medicine team. This role involves providing comprehensive healthcare to individuals and families, with a focus on health promotion and disease prevention for patients of all ages. The position is full-time (1.0 FTE) with 36 patient contact hours per week, typically working 4 days a week. The role includes coverage at the Fosston Clinic (3 days/week) and Oklee or Bagley Clinic (1 day/week), with an average of 15-18 patients per day. There is also a requirement for WIC coverage one Saturday per month. The ideal candidate will have the ability to work independently and possess at least 2 years of practice experience in Family Medicine. As a member of the Care Team, the NP/PA will utilize Essentia Care Processes and collaborate with other team members to ensure continuity of care and achieve team objectives. High-level customer service skills are essential for positive interactions with patients and colleagues.

Requirements

  • Master’s or Doctorate degree in Nursing or Bachelor’s or Master’s degree in Physician Assistant Studies from an accredited program.
  • Family Medicine and ER advanced practice experience required.
  • Current licensure as a registered nurse in appropriate state (if NP).
  • Current licensure as a Nurse Practitioner or Physician Assistant in appropriate state.
  • Current certification in CPR.
  • National certification, in applicable area.
  • Successful completion of Essentia Health credentialing process prior to practice.
  • Ability to work independently.
  • 2+ years of practice in Family Medicine preferred.

Responsibilities

  • Works independently and in collaboration with physicians and other health care professionals.
  • Provides health care to individuals and families, emphasizing health promotion and disease prevention.
  • Cares for patients ranging in age from newborn to elderly.
  • Works collaboratively with other care team members to achieve Care Team objectives and provide continuity of patient and family care.
  • Maintains a high level of customer service skills to establish and enhance positive relationships with patients, co-workers, and others.
