Requirements

• Active Arizona Nurse Practitioner license
• Nationally board certified
• Hold Pediatric Certification
• Must be registered Medicaid
• Comfortable conducting comprehensive health assessments
• Comfortable identifying care gaps
• Comfortable working with pediatric populations
• Strong assessment and clinical decision-making skills
• Comfortable traveling locally for scheduled in-home visits
• Enjoy working independently
• Bring warmth, professionalism, and excellent communication skills

Responsibilities

• Conduct comprehensive preventive health assessments during scheduled in-home visits.
• Identify care needs and close important gaps in care.
• Connect members to appropriate ongoing care.
• Focus on the member and deliver a thoughtful, high-quality assessment.
• Travel locally for scheduled in-home visits.
• Work independently while being part of a supportive clinical organization.
• Bring warmth, professionalism, and excellent communication skills into every member's home.
